If you are a survivor and are not able to view your kit information, or have received a login error, it may be that your kit has not yet been entered into the system.
Please contact the health care facility where the evidence collection was performed to get your information.
If you are a portal user, contact your local Track-Kit administrator.
If you cannot identify your local Track-Kit administrator, contact Track-Kit support by phone at 410-599-4031 or by email at track-kit.support@invitahealth.com.
Track-Kit support personnel can tell you the name(s) of your Track-Kit administrators. Support personnel do not have the ability to reactivate your account.
