If you have forgotten your password, please check the following:
Are you on the correct Track Kit site?
Does the URL for this webpage contain the two-letter abbreviation for your state or territory? If not, you need to verify and enter the URL for the Track-Kit system for your state.
Is your account disabled?
If you have not logged into Track-Kit for 120 days or more, your account is automatically disabled, as per the State's policy.
Contact your local Track-Kit administrator for account reactivation.
If you cannot identify your local Track-Kit administrator, contact Track-Kit support by email at track-kit.support@invitahealth.com.
Track-Kit support personnel can identify your Track-Kit administrators, but they do not have ability to reactivate your account.
After checking the above, if you have forgotten your password:
If you are a portal user, request a password reset by entering your username (email address) for your account.
Note: Password reset expires after 72 hours.
If you are a survivor, contact the medical facility where the evidence was collected or your law enforcement agency.
